Welcome to the A-Med Health Care Online Forms Help page. In it's continuing effort to provide technology-based solutions, A-Med has developed forms in PDF (portable document format) for your convenience.

The PDF forms are basically duplicates of A-Med's standard paper forms. Entry fields have been added which will allow filling out the forms using your personal computer, either online or offline. PDF forms are compatible with a wide variety of computer operating systems, including Windows, Apple/Mac, UNIX, SunOS, etc. The only requirements are to have Adobe Acrobat Reader installed on the computer you’re using and an Internet connection. The Adobe Acrobat reader can be obtained by clicking on the icon below:

The program is easy to install and takes only a few minutes. The program is used widely on the Internet and is therefore a handy utility to have.

PDF Forms - Online or Offline

The PDF forms can be used two ways; online and offline. Using them offline has the added benefit of having the file located on your computer so that it can be filled out and printed anytime. To use online, access the desired form, fill in the requested information and print the form from the last page.

To download to your computer, on the index page, right mouse click on the link to the desired form and choose "Save Target As" option. A dialog box will open so that you may select a location to store the file. Once downloaded, double-click on the file to open it, then fill-in the requested information and print it out from the last page. Or open the file online and click the "Save File" icon (see below).

The easiest way to fill out a PDF form after it is open in your browser is to:

1. Left mouse click on the first line you want to fill in
2. Type the required information
3. Press the "Tab" key to move to the next line to be filled in
4. At the end of the page, clicking the "next page" button will take you to the next page
5. Check boxes can be checked or unchecked by simply clicking on them
6. After filling in the form, check for accuracy and then click the print button.

Helpful Hints

If an error is made, simply click on the field in error, correct the information using the delete key to erase the erroneous data. when the print dialog appears, make two copies so that you have one for your records.

Acrobat Help 

After successfully installing Adobe Acrobat Reader on your computer, the forms will open in a special window within your browser's window. It has several tools which make viewing and navigating the forms easier. Acrobat also makes saving the forms to your computer as easy as clicking on the "Save File" (small picture of a floppy disk) icon. Click on the above snapshot to see a description of some of the other helpful tools.
